Pathways
The concept for Pathways is to allow an individual to age in place without ever having to move or relocate to a different area of the community. A resident with mild cognitive impairment resides in their apartment and participates in all activities of the communities without restriction. As dementia progresses and a secure environment becomes necessary with more assistance, the resident can remain in the same apartment and becomes parts of the Pathways Program. To keep residents secure in the facility, Pathway’s residents wear a Wander Guard Bracelet which restrict their exit from the facility.

Best Friends Approach to Memory Care
The Best Friend approach was developed by Virginia Bell and David Troxel while they were working at the University of Kentucky Alzheimer’s Disease Research Center. The premise of their approach is that what a person with dementia needs most of all is a friend a “Best Friend”. This can be a staff member who empathizes with their situation, remains loving and positive and is dedicated to helping the person feel safe, secure and valued.

Health & Wellness
Health and Wellness – Registered, Nurse Practitioner, Physician, Psychiatrist and Podiatrist along with Trained staff in Best Friends approach to Dementia Care are available to provide the best care to Autumn Care Memory Lane residents and support to their families. In addition, on-site gym with physical, occupational and speech therapist provide Medicare approved therapy whenever deemed necessary.
Nutrition & Hydration
Registered Dietician develops and monitors resident’s diets for enjoyment and enhance nutritional intake and hydration. The dining experience is designed to encourage and stimulated appetite with bright and colorful china and easy to handle utensils. Snacks are available anytime with the open kitchen.
Hygiene & Grooming
Staff is trained not only to assist residents with activities of daily living but to also make sure they look their best and help them maintain a sense of well-being.
Activities & Socialization
Pathway staff strives daily to engage residents through an array of activities designed to use and maintain large and small motor skills and also have fun and relax. Outings are also regularly scheduled and entertainers engaged weekly.
