Peter Falk, LNHA | Autumn Care, Assisted Living and Home Services in Knoxville


Chief Executive Officer

Peter Falk is Autumn Care’s Founder and CEO. He has a degree in health care administration from the University of Seventh Day Adventist, is a licensed Nursing Home Administrator, a Certified Nursing Assistant, and a licensed building contractor.

Peter has worked for some of the best rehabilitation and assisted living corporations in the country and has vast amounts of knowledge and experience in caring for seniors. With an unlimited depth of resources provided by the entire organization, Peter is very involved and continues to ensure Autumn Care is able to offer excellent services in each community that we serve.

Chief Operating Officer

Mari oversees every aspect of the company which includes the quality of service, amenities, care, and programming for each and every Autumn Care Assisted Living Community. Mari has over 10 years of experience in the industry and has been with the company since its inception in 2008.

Chris Falk

Finance & Accounting Director

Chris has lived in Knoxville most of his life and is a Webb School of Knoxville alumni. He is a graduate of the Daniels School of Business at Denver University with a minor in history. He joined Autumn Care in 2015 and is responsible for accounting and finance operation of the company. In his spare time, Chris enjoys golf and is a skilled musician.

Brandy Manifold

Executive Director Karns

Brandy Manifold begins her new role as Executive Director of Karns after holding the position of Community Relations Director for all three Autumn Care Communities for the past two years. She is a veteran with a degree in Operations Resource Management from the United States Air Force. While stationed on the west coast she had the opportunity to attend Concordia University and The University of Phoenix where she studied business management. Brandy and her family moved to Knoxville after her husband retired from the Air Force in 2018. After 27 years of service they are excited to call Tennessee home. She comes to us with years of experience serving in senior communities and has a passion for serving seniors. In 2019 she received her Best Friends Approach Master Trainer Certification BF-CMT to better manage daily interactions of staff working with residents with dementia diagnosis. Building relationships with her residents and families is her favorite part of working at Autumn Care. Her husband of 23 years still enjoys flying full time and both of her daughters attend university, pursuing degrees in the medical field. In her spare time, Brandy loves to entertain family and friends at her home by the lake.

Robert Sawicki

Manager of Outpatient Therapy Services

Robert is a graduate of the  University of Tennessee with a BS in Human Resources and a focus on Corporate Ethics, Integrity, and Compliance. He received his degree in Physical Therapy Studies from South. Robert has been a licensed Physical Therapist Assistant in the state of Tennessee from 1999 – Present, and has over 20 years of experience in the Tennessee Health Care system.

Carol HUddleston

Executive Director- Farragut

Carol Huddleston is a Registered Nurse; she obtained her Bachelors Degree from King University. She is married to Retired Sargent Preston Huddleston and the mother of 2 daughters and a son. Carol has spent over 20 years taking care of the Geriatric population. In her spare time she likes reading and taking care of her 4 grandchildren.

Yvonne Blair

Director of Nursing – Farragut

Yvonne was born and raised in Knoxville, Tennessee. She has over 20 years of experience in healthcare working with the elderly. She started as a Certified Nursing Assistant and worked for 10 years, then obtained her nursing license through Tennessee Technology Center in 2006.She then worked as a Certified Nursing Assistant Instructor for 9 years. She is a mother of two wonderful daughters, both of which are also in the nursing field, and a proud grandmother of four.

Kayla Webb

Director of Nursing – Karns

Kayla was born and raised in Knoxville. She obtained her nursing license thru Tennessee Technology Center. Kayla has ten years of geriatric experience. She loves caring for her patients and enjoys working with their families. Kayla and her husband, Kevin have two beautiful daughters. She enjoys going to church, where her husband is pastor, and spending time with her family.

Rae Aguero

Activities Director – Karns

Rae has a heart for the older adult and creates an environment that is at once inviting and stimulating. She creates, finds and arranges outings and events for residents like performances, clubs, fitness classes, day trips to unique places, and much more. Rae joined Autumn Care in 2011.

Brandie Knuckles

Director of Nursing – West Knoxville

Brandie is from Knoxville, TN. She has 15 years of experience working in healthcare with the Senior Community. Brandie takes compassion in caring for others which is what drew her to Nursing. Brandie began her career in healthcare as a certified nursing assistant then decided to further her education as a Nurse. Brandie has a passion for nursing and she takes pride in caring for her residents and interacting with their families. Brandie is very family oriented so in her spare time she loves spending time with her husband and their three beautiful children.

Alexendria Creswell

Alexandria Creswell

Activities Director – West Knoxville

Alexandria has lived in East Tennessee the majority of her life. She and her husband find great joy in raising their three children here in our beautiful Tennessee Valley. Alexandria’s journey in health care began with her desire to find a spiritually rewarding career. She worked as a caregiver in the homes of East Tennessee’s elderly for years and continued on as a caregiver at Autumn Care West. Thereafter her career carried on as a Scheduling and HR Coordinator in the Home Care field. Her passion continued to grow for this cause and her mission became to spread joy and positivity to the lives of others. She feels her skill set and love for the elderly can best be utilized as an Activities Director where she can make one on one connections providing love, care and companionship to each resident

Hannah Green

Hannah Green, NP

Wound & Foot Care

Hannah earned her bachelor’s in nursing from King University and started her nursing career at the University of Tennessee Medical Center with the Vascular Surgery and Transplant service. There she worked with these complex patients for several years. She decided to advance her learning and become a Nurse Practitioner earning her degree from Lincoln Memorial University.

Hannah knows that it is a privilege to provide care for people in her community and she believes that the best approach to healthcare is focused on the patient from a holistic perspective.

Hannah is a Knoxville native, and loves all things rocky top! When she isn’t working, she enjoys spending time with her husband, cuddling with her dog, and soaking up some sun.

Ashton Waston

Ashton Watson, FNP

Ashton Watson is a certified Family Nurse Practitioner. She began her nursing career in 2010, earning her Bachelor of Science Degree in Nursing from Tennessee Wesleyan University in Athens, TN. She later earned her Masters of Science Degree in Nursing from Carson Newman in the Family Nurse Practitioner program. She is certified by the American Association of Nurse Practitioners, and licensed by the state of Tennessee as an Advanced Practice Nurse. Ashton started her career in critical care medicine. She went on to become a Nurse Practitioner and began her career in the field of geriatrics. She currently works in skilled nursing facilities as well as assisted living facilities. Ashton is married and has two beautiful children. In her free time she enjoys spending time with her family, Tennessee football, and traveling as much as possible!

Deaver Shattuck

Deaver Shattuck

Deaver Shattuck was born in Knoxville and raised in Clinton. A charter member of the Society of Hospital Medicine, he was the original hospitalist at Blount Memorial Hospital where he still practices. He founded and is medical director of Blount Hospitalists. He has served at BMH as chief of staff and spearheaded several quality initiatives: he championed the hospital’s EMR implementation, established the sepsis team and led the effort for the hospital to become a primary stroke center. He continues to serve as medical director for the stroke program.

With the business success of Blount Hospitalists he and his friend Dr. Teresa Catron co-founded what has grown to be the largest private geriatric group in Tennessee, Blount Senior Care Partners. This group provides complete care in a unique model from several venues: their geriatric specialty clinic, all of the long term care, rehab, and assisted living facilities in Blount County as well as Blount Memorial Hospital. They have developed service lines not just in specialty geriatric care, but also foot and wound care and the only fully integrated geriatric focused mental health services in the area.

Heartened by the growth and the clinical excellence of these groups he decided to expand their care model into Knoxville with HouseCall Primary Care.

Shattuck serves on Tennessee’s Stroke Best Practices Task Force and on the church council at St. John’s Lutheran. He often presents on stroke care and on our community’s addiction epidemic. His wife Christie and he recently celebrated their 25th anniversary and have 5 children.

Jenna Frost

Jenna Frost, NP

Jenna is a Psychiatric-Mental Health Nurse Practitioner. She started her clinical career as RN and team leader on a med-surg unit. Her time as an RN helped solidify her passion for the geriatric population. She felt a calling to care for & be the voice for those who cannot always speak for themselves.

Jenna graduated with her master’s in nursing in 2018 from Lincoln Memorial University. She provides care in local nursing homes and assisted livings, enabling her to help seniors maximize their mental health. Jenna is a Knoxville native, born and raised. In her free time, Jenna enjoys spending time with her husband and family. She enjoys reading, going to church, and cheering on the Kentucky Wildcats!


Carol Long


Carol Long

Physician – Farragut

Dr Carol Long was born and raised in Ohio. She went to Wittenburg University for her undergraduate and graduated from Ohio State University of Medicine. She served 3 years as a physician in the Airforce in Maine and was a Major when she discharged. After this, she opened a solo primary care practice in Maryville, TN, where she cared for older adults, primarily, for 24 years.

In her spare time, Dr Long volunteers for the Animal Shelter, Kiwanis and is a Board Member @ Compassion Counseling. She likes making pottery and loves to fish, but confesses she is not good at catching any. At the end of the day.. spending time with her 2 cats, Spooky & Chessie, brings her much joy.
